Our patients come first, so we appreciate your referrals.
Who can refer a patient?
It all begins with a referral. Amistad Homecare, Inc. accepts referrals from physicians, hospitals, skilled nursing & rehab facilities, community service programs, and family & friends.
What is needed?
In order to initiate the referral process, we will need the following patient information:
Name, address, and phone number.
Insurance information.
Pertinent medical information such as a History and Physical (H&P) and specific orders, if any.
Signed admission order by the certifying physician.
Signed and completed the Attestation form.
